Mapping safety-critical responsibilities to roles
A North Sea operator needed to show which roles carried its safety-critical responsibilities, and to check each one against the sentence in its major accident hazard documents that assigns it.

The situation
The operator's management system had grown over years of change. Responsibilities were written in different ways across its documents, and some documents still named roles that had since been renamed or removed.
The team needed a view of who held each safety-critical responsibility that they could check line by line.
What Synapse did
Synapse read the 77 documents the operator supplied and extracted the responsibilities written into them. It mapped each one to a role, starting with 38 safety-critical senior leadership roles, and kept the verbatim sentence and document reference with every assignment.
Each documented activity was rated for safety criticality, and the ratings were rolled up for each role. The evidence also showed removed and renamed roles that still carried responsibilities.

Safety criticality, traced to the sentence
For the Offshore Installation Manager, 87 of the 210 responsibilities Synapse found score SC 3 or above. SC is the safety-criticality score, from 0 to 5, with 5 the highest. Filtering on it gives a reviewer a short list to check first, and each entry opens at the sentence it came from.

Control all incident response activities on the platform
Overall command of all emergency situations within the 500m zone is the responsibility of the Offshore Installation Manager (OIM). The OIM is required to control all incident response activities on the platform and is responsible for co-ordinating the activities of all rescue services and vessels, both air and sea, assisting with the emergency.
